Job Summary
A company is looking for a Care Manager to provide care management services and develop tailored care plans for individuals.
Key Responsibilities
- Develop and maintain positive relationships with members, families, and care teams to enhance health outcomes
- Conduct comprehensive health assessments and coordinate care team meetings to implement and evaluate care plans
- Monitor service delivery, ensure medication reconciliation, and document all interactions and activities electronically
Required Qualifications, Training, and Education
- Two years of experience as a Care Manager, Case Manager, or Care Coordinator preferred
- Must meet all agency and state requirements for pre-employment
- A license or permit issued by a governing board regulating a human service profession, or a relevant degree with specified experience
- Experience working with individuals with intellectual and developmental disabilities (IDD) or traumatic brain injury (TBI)
- For care managers serving members with long-term support services (LTSS) needs, two years of prior coordination and care management experience is required
