North Carolina Licensed Care Manager

Job Summary

A company is looking for a Care Manager to provide care management services in accordance with state requirements.

Key Responsibilities

• Develop and monitor Tailored Care Management care plans and Individual Support Plans (ISPs) for assigned caseloads
• Engage with members, families, and various stakeholders to address health-related needs and ensure comprehensive care
• Document all interactions and care team activities electronically in a timely manner

Required Qualifications, Training, and Education

• Two years of experience as a Care Manager, Case Manager, or Care Coordinator preferred
• A license or certification in a relevant human service profession, or a Master's or Bachelor's degree in a human service field with specified experience
• Must meet all agency and state requirements for pre-employment
• Experience working with individuals with intellectual and developmental disabilities (IDD) is required
• For those serving members with LTSS needs, two years of prior coordination and care management experience is required
