Job Summary

A company is looking for a Case Manager.

Key Responsibilities

• Coordinate care, resources, and services for members to achieve optimal clinical and resource outcomes
• Develop and monitor member-centric care plans tailored to individual needs and barriers
• Educate members and facilitate proactive interventions to improve health outcomes and manage costs

Required Qualifications

• Registered Nurse (RN) with 3 years of transplant clinical and/or transplant case management experience
• Valid and applicable clinical license (NC or compact multi-state licensure)
• Additional specialty certification (i.e., CCM) required within 2 years of employment
