Job Summary

A company is looking for a Case Manager.

Key Responsibilities

• Coordinate care, resources, and services for members to achieve optimal clinical outcomes
• Develop and monitor member-centric care plans tailored to individual needs and barriers
• Document all aspects of the care plan from assessment to evaluation of outcomes

Required Qualifications

• Registered Nurse (RN) with 3 years of clinical and/or case management experience
• Valid and applicable clinical license (NC or compact multi-state licensure)
• Additional specialty certification (i.e., CCM, CDCES) may be required within 2 years
• Experience in disease management and chronic condition care is a plus
• Certified Diabetes Educator (CDE) is a bonus
