Job Summary

A company is looking for a Case Manager - Medicare Advantage.

Key Responsibilities

• Coordinate care, resources, and services for members to achieve optimal clinical and resource outcomes
• Develop and implement member-centric care plans, addressing barriers and utilizing community resources
• Monitor member progress, provide education, and document all aspects of care management

Required Qualifications

• Registered Nurse (RN) with 3 years of clinical and/or case management experience
• Valid and applicable clinical license (NC or compact multi-state licensure)
• Additional specialty certification (e.g., CCM, CDCES) may be required for some roles
• Must obtain relevant certification within 2 years of employment if applicable
