Job Summary

A company is looking for a Case Manager.

Key Responsibilities

• Coordinate care, resources, and services for members to achieve optimal clinical and resource outcomes
• Develop and implement member-centric care plans, addressing barriers to achieving health goals
• Monitor member progress and provide education to encourage proactive health management

Required Qualifications

• Registered Nurse (RN) with 3 years of clinical and/or case management experience
• Valid and applicable clinical license (NC or compact multi-state licensure)
• Additional specialty certification (e.g., CCM, CDCES) may be required for some roles
• Ability to perform comprehensive assessments and evaluate care across the continuum
• Experience in documenting care plans and outcomes effectively
