North Carolina Licensed Case Manager

Job Summary

A company is looking for a Case Manager.

Key Responsibilities

• Coordinate care, resources, and services for members to achieve optimal clinical and resource outcomes
• Develop and implement member-centric care plans, addressing barriers and utilizing community resources
• Monitor and evaluate member progress, adjusting plans as needed and documenting all aspects of care

Required Qualifications

• Registered Nurse (RN) with 3 years of clinical and/or case management experience
• Valid and applicable clinical license (NC or compact multi-state licensure)
• Additional specialty certification (i.e., CCM, CDCES) may be required for some roles
• Ability to perform comprehensive assessments and collaborate with healthcare teams
• Experience in evaluating care provided across the continuum of care
