North Carolina Licensed Case Manager

Job Summary

A company is looking for a Commercial Case Manager to support care coordination initiatives for a health insurance organization.

Key Responsibilities:

• Deliver telephonic case management services to members with diverse clinical needs
• Conduct comprehensive assessments and develop, implement, and monitor individualized care plans
• Partner with internal clinical staff and external healthcare providers to promote optimal health outcomes

Required Qualifications:

• Active RN license (NC or compact multi-state), maintained in good standing
• Minimum of 3 years of clinical and/or case management experience
• Previous telephonic case management experience is mandatory
• Background managing a variety of case types across multiple clinical specialties
• Knowledge of health insurance operations is a plus
