Job Summary
To support a patient-centered approach in healthcare, the full-time Healthcare Case Manager will coordinate care, conduct assessments, and develop individualized care plans while working remotely in North Carolina or South Carolina.
Key responsibilities
- Coordinate care and resources for members as part of a multidisciplinary team
- Conduct comprehensive assessments and collaborate on care plans with members, families, and providers
- Monitor member progress and adjust care plans as necessary to improve health outcomes
Required qualifications
- Current RN, RD, or LCSW with 3+ years of clinical and/or case management experience (or MSW with 5+ years in some cases)
- Active, valid clinical license in North Carolina or a compact state
- Strong care coordination and clinical decision-making skills
- Experience with multidisciplinary teams and community resources
- Knowledge of utilization management and medical necessity criteria
