Job Summary
To enhance clinical outcomes in a remote setting, the North Carolina Licensed Case Manager will coordinate healthcare services, conduct comprehensive assessments, and develop individualized care plans while engaging with members and care teams.
Key responsibilities
- Conduct comprehensive clinical assessments and create tailored care plans addressing medical, behavioral, and social needs
- Coordinate services across various healthcare settings, including hospitals and home-based care
- Maintain ongoing communication with members and care teams to evaluate progress and adjust care strategies as needed
Required qualifications
- Active North Carolina RN license or Compact RN license in good standing
- Minimum of 3 years of clinical nursing and/or case management experience
- Prior telephonic case management experience within a health insurance or managed care organization
- Experience supporting Medicare and/or Medicaid populations, with a preference for behavioral health case management
- Strong skills in care coordination, assessment, and documentation, with the ability to work independently in a remote environment
