Job Summary
To support a leading organization in the managed care and health insurance industry, a remote North Carolina Licensed Case Manager will conduct comprehensive clinical assessments, develop individualized care plans, and coordinate healthcare services to improve clinical outcomes.
Key responsibilities:
- Conduct comprehensive clinical assessments and develop individualized care plans addressing medical, behavioral, and social needs
- Coordinate services across various healthcare settings and maintain ongoing communication with members and care teams
- Educate members on treatment options, community resources, and preventative health measures while documenting case activities accurately
Required qualifications:
- Active North Carolina RN license or Compact RN license in good standing
- Minimum of 3 years of clinical nursing and/or case management experience
- Prior telephonic case management experience within a health insurance carrier or managed care organization
- Experience supporting Medicare and/or Medicaid populations, with a preference for behavioral health case management
- Strong care coordination and assessment skills with the ability to work independently in a remote setting
