Job Summary
To enhance healthcare coordination, the remote North Carolina Licensed Case Manager (RN) will conduct clinical assessments, develop care plans, and maintain communication with members and care teams to improve clinical outcomes.
Key responsibilities
- Conduct comprehensive clinical assessments and create individualized care plans addressing medical, behavioral, and social needs
- Coordinate services across various healthcare settings and maintain ongoing communication with members and care teams
- Educate members on treatment options, benefits, and community resources while documenting case activities and ensuring compliance
Required qualifications
- Active North Carolina RN license or Compact RN license in good standing
- Minimum of 3 years of clinical nursing and/or case management experience
- Prior telephonic case management experience within a health insurance carrier or managed care organization
- Experience supporting Medicare and/or Medicaid populations, with a preference for behavioral health case management
- Strong skills in care coordination, assessment, and documentation, with the ability to work independently in a remote setting
