North Carolina Licensed Case Manager, RN

Job Summary

A company is looking for a Remote Case Manager, RN.

Key Responsibilities

• Collaborate with a health care team to monitor utilization patterns and facilitate appropriate health care delivery
• Conduct comprehensive assessments of members' health status, including medical records and support needs
• Coordinate treatment plans and discharge plans with primary care physicians and family members

Required Qualifications

• RN or LPN with a North Carolina license required
• Prior telephonic case management experience is a plus
• 3+ years of inpatient care experience
• Critical care experience, including med-surg and ICU
• Must reside in the EST time zone
