North Carolina Licensed Case Manager RN

Job Summary

A company is looking for a Case Manager Registered Nurse (RN) for a fully remote opportunity.

Key Responsibilities:

• Collaborate with a multidisciplinary healthcare team to monitor utilization patterns and facilitate appropriate care delivery
• Conduct comprehensive assessments of members' health status, including clinical history and support needs
• Coordinate with healthcare providers to develop and implement treatment and discharge plans

Qualifications:

• Active RN license (North Carolina or compact state required)
• 3+ years of inpatient care experience, including critical care (e.g., Med-Surg, ICU)
• Prior telephonic case management experience is preferred
• Must reside in the Eastern Time Zone
