Job Summary

Leading the development of a comprehensive chronic disease management strategy, the full-time NC ROOTS Regional Head of Chronic Disease Management Program Development will design and operationalize programs focused on high-burden conditions while working remotely within Region 5 of North Carolina.

Key responsibilities
  • Develop a regional chronic disease management strategy aligned with NC ROOTS priorities
  • Design workflows for care management, patient education, and transitions of care
  • Align chronic disease programs with community-based services and track key performance indicators
Required qualifications
  • Associate's degree in Nursing with three years of relevant experience, or Bachelor's degree in Nursing with one year of experience
  • Fully licensed as a Registered Nurse (RN) by the North Carolina State Board of Nursing
  • Must possess a valid driver's license
  • Residence in North Carolina within Region 5 of Trillium's catchment area
  • Experience in designing or implementing care management or chronic disease programs
