Job Summary
Leading the development of a comprehensive chronic disease management strategy, the full-time NC ROOTS Regional Head of Chronic Disease Management Program Development will design workflows and align programs with community services to improve health outcomes for high-burden conditions while working remotely in North Carolina.
Key responsibilities
- Lead the development of a regional chronic disease management strategy aligned with NC ROOTS priorities
- Design workflows for care management, patient education, and transitions of care
- Define and track key performance indicators related to chronic disease outcomes and care management
Required qualifications
- Associate's degree in Nursing with three years of relevant experience, or Bachelor's degree in Nursing with one year of experience
- Fully licensed by the North Carolina State Board of Nursing as a Registered Nurse (RN)
- Valid driver's license and residency in North Carolina within specified counties
- Experience designing or implementing care management or chronic disease programs
- Equivalent combination of education and experience may be considered
