Job Summary
To support Medicare quality initiatives, the remote North Carolina Licensed Clinical Pharmacist will engage with Medicare members, conduct Comprehensive Medication Reviews (CMRs), and collaborate with providers to improve medication adherence and health outcomes.
Key Responsibilities
- Conduct Comprehensive Medication Reviews (CMRs) with eligible Medicare members to enhance medication understanding and adherence
- Perform proactive outreach to address barriers to medication adherence and collaborate with pharmacies and providers to resolve adherence gaps
- Provide medication counseling and education to Medicare members while maintaining accurate documentation for compliance with quality standards
Required Qualifications
- Bachelor's degree in Pharmacy or PharmD preferred
- Active North Carolina pharmacist license and residency
- Minimum of 3+ years of experience in a related field
- Strong knowledge of medication therapy management and adherence strategies
