Job Summary

To support a dynamic team focused on improving patient outcomes, the full-time remote North Carolina Licensed Clinical Pharmacist will conduct outreach to Medicare members, collaborate with healthcare professionals, and optimize medication adherence.

Key responsibilities

• Conduct outreach to Medicare members to identify and resolve barriers to medication adherence
• Collaborate with pharmacies and providers to close adherence gaps for unengaged or high-need members
• Complete Comprehensive Medication Reviews (CMRs) to optimize therapy and educate members

Required qualifications

• Active, current NC state pharmacist license
• Bachelor's degree or advanced degree in Pharmacy
• 3+ years of experience in a clinical, retail, or managed care setting
• Strong communication and patient engagement skills
• Ability to collaborate across providers, pharmacies, and internal teams
