Job Summary

A company is looking for a Director of Community Integration Strategic Engagement.

Key Responsibilities
  • Design and lead strategies for healthcare and social service integration
  • Build and maintain collaborative relationships with community stakeholders
  • Oversee clinical initiatives addressing social determinants of health
Required Qualifications
  • Bachelor's degree in social work, public administration, health services, or related field
  • Seven years of experience in ADA/Olmstead integration, including four years in a supervisory role, or a Master's degree with five years of experience including two years in a supervisory role
  • Certified Care Management (NACCM, CCMC, ACCM) required within two years of hire
  • Experience with NC Medicaid Managed Care and marginalized populations
  • Knowledge of federal and state regulations related to disability services
