Job Summary

To support a pioneering healthcare team, the full-time North Carolina Licensed Family Nurse Practitioner will conduct virtual wellness visits, health assessments, and provide comprehensive patient care coordination in a remote setting.

Key responsibilities

• Conduct virtual wellness visits and health assessments for patients in their homes
• Collaborate with in-home clinical team members to evaluate patient needs and arrange necessary services
• Provide comprehensive care coordination, including preventive care interventions and medication management

Required qualifications

• Board Certified as a Family Nurse Practitioner
• Active Family Nurse Practitioner License in North Carolina
• Demonstrated professionalism, integrity, and compassion in patient care
• Technologically savvy with experience using digital tools for patient care and documentation
• Ability to work autonomously and adapt to a dynamic healthcare environment
