Job Summary

A company is looking for an LPN Coach to support patients post-discharge in a remote care setting.

Key Responsibilities
  • Conduct post-discharge follow-up calls to assess patient needs and ensure successful care transitions
  • Schedule follow-up appointments with primary care providers and specialty clinics
  • Document patient interactions and care coordination activities in the electronic medical record
Required Qualifications, Training, and Education
  • Current, unrestricted LPN license for the states in the company's footprint, required
  • High School Diploma or GED, required; 2 Year / Associate Degree, preferred
  • Minimum three years clinical experience in a hospital, home health, or community setting, required
  • EPIC experience, highly preferred
  • Broad clinical knowledge base of multiple conditions and expected outcomes
