North Carolina Licensed LTSS Care Manager

Job Summary

A company is looking for an LTSS Care Manager.

Key Responsibilities

• Facilitate person-centered planning meetings to coordinate care and services
• Identify and assist participants in accessing needed LTSS services and other supports
• Complete assessments and inform participants about available services and care options

Required Qualifications

• Must meet Qualified Professional (QP) status
• Fully licensed by the NC governing board or a licensed RN with specific experience
• Master's degree in a human service field with one year of relevant experience, or a bachelor's degree with two to four years of relevant experience
• Minimum of two years of prior LTSS and/or HCBS coordination experience
• Must have a valid driver's license and reside in NC
