Job Summary
To support a growing healthcare team, the full-time Remote North Carolina Licensed Nurse Care Manager will coordinate care for members with chronic conditions, conduct assessments, and develop individualized care plans while engaging in telephonic communication and documentation.
Key responsibilities
- Conduct comprehensive assessments to evaluate member needs and connect them with resources
- Develop and implement individualized care plans, monitoring member progress and advocating for necessary services
- Document care management activities accurately and participate in interdisciplinary team meetings to discuss member care strategies
Required qualifications
- Active & unrestricted North Carolina RN license or compact State license
- 2-3 years of clinical practice experience in a healthcare or managed care setting
- 3 years of case management experience
- High School Diploma or equivalent; Associate's Degree preferred
- Proficiency in navigating multiple healthcare technology tools
