Job Summary
Providing comprehensive nursing care, the full-time North Carolina Licensed Nurse Case Manager will assess patients, develop treatment plans, and coordinate care for Department of Defense beneficiaries, while working remotely.
Key responsibilities
- Collect and analyze health data to create time-sensitive treatment plans for case-managed patients
- Conduct telephone assessments and establish patient care priorities to ensure effective healthcare delivery
- Oversee discharge planning and facilitate referrals to appropriate healthcare providers
Required qualifications
- Current, unrestricted Registered Nurse or Licensed Clinical Social Worker license in the U.S
- Minimum of two years of nursing or case management experience with certification from a recognized organization
- Ability to obtain a Secret or Top Secret clearance
- Knowledge of patient education principles and management of complex medical issues
- Current certification in Basic Cardiac Life Support (BCLS) by the American Heart Association
