Job Summary
Providing comprehensive virtual primary care, the full-time North Carolina Licensed Nurse Practitioner will manage a patient panel through video visits, delivering assessment, diagnosis, and treatment while collaborating with a multidisciplinary team in a remote setting.
Key responsibilities:
- Deliver high-quality virtual primary care through video visits, including assessment, diagnosis, treatment, and patient education
- Manage a panel of patients, providing comprehensive care across acute, chronic, and preventive needs
- Collaborate with the Care and Case Management team to develop care plans and support successful outcomes
Required qualifications:
- Minimum of four years experience delivering primary care in an ambulatory/outpatient family medicine or internal medicine setting
- Active, unrestricted Nurse Practitioner and RN license in state of residence; ability to be licensed in additional states as required
- Current and active board certification by a national certifying body through ANCC or AANP
- Demonstrated ability to provide empathetic, compassionate care and support to patients and families
- Strong proficiency with technology and electronic health platforms; experience with telehealth preferred
