Job Summary
To support a mission-driven clinical team, the full-time North Carolina Licensed Nurse Practitioner will conduct telehealth assessments, manage chronic conditions, and emphasize preventative care while working remotely.
Key responsibilities
- Conduct primarily telehealth video visits and occasional in-home assessments for members
- Perform comprehensive evaluations and provide evidence-based care focused on prevention and chronic condition management
- Collaborate with care team members to identify care gaps and develop individualized care plans
Required qualifications
- Active Nurse Practitioner (APRN/NP) license
- 3+ years of outpatient clinical practice, preferably in primary care or geriatrics
- Active BLS certification at time of hire
- Comfort with delivering care via video telehealth and using electronic documentation systems
- Willingness to obtain additional licenses if necessary within the first 90 days
