North Carolina Licensed Nurse Practitioner

Job Summary

To support a growing Lifestyle Medicine team, the full-time North Carolina Licensed Nurse Practitioner will conduct focused virtual visits and occasional in-person consultations, collaborating with patients to create personalized health action plans while utilizing a hybrid care model that emphasizes behavior change and whole-person health.

Key responsibilities:

• Conduct focused virtual and occasional in-person visits to assess patients' health habits and life circumstances
• Collaborate with patients to develop clear, practical Lifestyle Medicine Action Plans tailored to their goals and constraints
• Utilize motivational interviewing techniques to empower patients in making meaningful health changes

Required qualifications:

• North Carolina Licensed Nurse Practitioner or Physician Assistant; South Carolina licensure optional
• DEA license
• American Heart Association BLS certification
• Broad foundation in primary care with a whole-person health perspective
• Expected to obtain board certification in Lifestyle Medicine within the first year of employment
