North Carolina Licensed Nurse Practitioner

Job Summary

To deliver world-class comprehensive care, the full-time North Carolina Licensed Nurse Practitioner will conduct primarily telehealth visits with opportunities for some home-based care, focusing on comprehensive assessments and coordinating care with a multidisciplinary team.

Key responsibilities:

• Conduct telehealth video visits and occasional home-based visits for comprehensive assessments and care coordination
• Perform diagnostic tests and reviews, including medication assessments and quality of care interventions
• Collaborate with the care team, including physicians and social workers, to develop and implement care plans

Required qualifications:

• Active RN and APRN license in North Carolina with certification required at hire
• Minimum of 3 years of experience in outpatient clinical practice
• Experience in primary care, geriatrics, or palliative care preferred
• Familiarity with telehealth platforms and conducting virtual visits
• Willingness to obtain multi-state licensure as needed
