Job Summary
To deliver world-class comprehensive care, the full-time North Carolina Licensed Nurse Practitioner will conduct primarily telehealth visits with opportunities for some home-based care, focusing on comprehensive assessments and coordinating care with a multidisciplinary team.
Key responsibilities:
- Conduct telehealth video visits and occasional home-based visits for comprehensive assessments and care coordination
- Perform diagnostic tests and reviews, including medication assessments and quality of care interventions
- Collaborate with the care team, including physicians and social workers, to develop and implement care plans
Required qualifications:
- Active RN and APRN license in North Carolina with certification required at hire
- Minimum of 3 years of experience in outpatient clinical practice
- Experience in primary care, geriatrics, or palliative care preferred
- Familiarity with telehealth platforms and conducting virtual visits
- Willingness to obtain multi-state licensure as needed
