Job Summary

Delivering advanced-level, patient-centered care, the remote North Carolina Licensed Palliative Care Clinician will focus on symptom relief, emotional support, and quality of life for individuals with serious illnesses while collaborating with interdisciplinary teams and families.

Key responsibilities
  • Provide palliative-focused direct patient care through telehealth visits, ensuring compassionate, evidence-based practice
  • Conduct comprehensive holistic patient assessments to create individualized care plans addressing physical, emotional, and spiritual needs
  • Lead goals-of-care discussions and assist in advance care planning, including completion of advance directives and life-sustaining treatment orders
Required qualifications
  • Currently licensed as a Nurse Practitioner (NP) or Physician Assistant (PA) in North Carolina
  • 2+ years of experience in palliative care, hospice, oncology, geriatrics, or a related field
  • Clinical competency in advance care planning and serious illness conversations
  • Solid primary palliative care skills with comfort in tertiary/specialized palliative care
  • Proven ability to manage complex caseloads and care plans in fast-paced settings
