Job Summary
To enhance care transitions for patients discharged from healthcare facilities, the full-time salaried North Carolina Licensed Population Health Manager will provide clinical expertise, support, and guidance remotely, focusing on patient assessments, care plan development, and coordination with interdisciplinary teams.
Key responsibilities
- Coordinate and implement assessments and care plans for patients, addressing their health and psychosocial needs
- Facilitate communication among care team members and engage patients and their support systems in decision-making processes
- Document all activities electronically and provide feedback to improve care management and address gaps in services
Required qualifications
- Bachelor's degree in a clinical field such as Nursing, Counseling, Social Work, or Allied Health
- Minimum of 3 years of clinical experience
- Current licensure as an RN, Licensed Clinical Social Worker, Licensed Professional Counselor, or Licensed Addiction Specialist in North Carolina
- ACM or CCM certification required within 3 years of hire date
