Job Summary

To enhance patient care during transitions, the remote North Carolina Licensed Population Health Manager will provide clinical expertise and support for patients discharged from healthcare facilities, focusing on care coordination, disease management, and patient engagement in a full-time capacity.

Key responsibilities
  • Coordinate assessments, care plans, and interventions for patients, considering their health, social, and environmental factors
  • Engage patients and their support systems in decision-making and provide targeted interventions to connect them with healthcare resources
  • Facilitate communication among interdisciplinary care teams to ensure seamless transitions and minimize fragmented care
Required qualifications
  • Bachelor's degree in a clinical field such as Nursing, Counseling, Social Work, or Allied Health
  • Three years of clinical experience required
  • Current licensure as an RN, Licensed Clinical Social Worker, Licensed Professional Counselor, or Licensed Addiction Specialist in North Carolina
  • ACM or CCM certification required within three years of hire date
