Job Summary

A company is looking for a Manager of Practice Transformation to oversee the Practice Transformation Specialists and ensure effective operations in compliance with policies and contractual requirements.

Key Responsibilities

• Manage and develop staff to ensure alignment with organizational goals and effective team performance
• Provide training and coaching to network providers, focusing on continuous quality improvement and data-informed care management
• Collaborate with internal and external stakeholders to enhance relationships and resolve issues effectively

Required Qualifications

• Master's degree in Social Work, Counseling, Healthcare Administration, Public Health, or Business Administration
• Three years of post-degree experience in healthcare quality, population health, or care management
• Previous supervision or management experience preferred
• Understanding of Health Information Technology (HIT) and its role in healthcare outcomes
• Moderate analytic knowledge and project management skills
