Job Summary
Providing nursing support to members with low-acuity, short-term clinical needs, the full-time North Carolina Licensed Resource RN will work remotely to conduct transitions of care assessments, perform clinical education, and evaluate members for higher levels of care management.
Key Responsibilities
- Conduct focused transitions of care assessments for members during inpatient stays or post-discharge
- Perform self-efficacy and condition-specific screenings, including behavioral health tools, to identify members needing additional support
- Collaborate with care team members to ensure proper medication reconciliation and address quality gaps in care
Required Qualifications
- Active North Carolina RN license or Compact RN license valid in North Carolina
- Graduate of an accredited school of nursing (R.N.)
- Minimum of 3 years of nursing experience
- Familiarity with North Carolina communities and healthcare resources
- Experience applying Motivational Interviewing and Trauma-Informed Care principles
