Job Summary
Supporting high-risk members through assessments and individualized care planning, the full-time North Carolina Licensed Senior Care Manager will manage complex caseloads and coordinate care remotely, with a schedule of Monday to Friday, 8:00 a.m. to 5:00 p.m., and flexibility available after training.
Key responsibilities:
- Develop and continuously assess personalized care plans while collaborating with healthcare providers to address members' unmet needs
- Monitor and revise care plans based on member status and progress towards goals, ensuring adequate person-centered care
- Educate members and their families on healthcare options, disease processes, and available services to improve care delivery
Required qualifications:
- North Carolina RN license or Compact State Licensure
- Degree from an accredited School of Nursing or a Bachelor's degree in Nursing
- 4 to 6 years of related experience in care management
- Experience in developing and implementing care plans for high-risk populations
- Ability to perform outreach through various channels, including telephonic and digital communication
