Job Summary
Managing a cohort of heart failure patients, the full-time North Carolina Licensed Telehealth Nurse Practitioner will evaluate patients virtually, create care plans, and optimize medication therapy while working remotely Monday through Friday.
Key responsibilities
- Virtually evaluate patients and assist in creating individualized care plans based on protocols
- Prescribe and titrate medications according to evidence-based guidelines and company standards
- Respond to alerts regarding patient vitals and manage clinical status changes within the APP's scope of practice
Required qualifications
- 3+ years as a Board-Certified Adult or Family Nurse Practitioner with experience in chronic care management, specifically in cardiology and heart failure
- Completion of an accredited NP program with national certification
- Active licensure in North Carolina and at least one additional state; preference for licensure in multiple states
- Prior experience managing heart failure patients and prescribing evidence-based therapies
- Previous telehealth experience is strongly preferred
