Job Summary
A company is looking for a Supervisor II-Transition Coordinator.
Key Responsibilities
- Supervise and develop staff to ensure compliance with policies and promote a positive work environment
- Oversee unit operations, including the development of clinical protocols and departmental goals
- Provide customer service by building strong relationships with providers and stakeholders
Required Qualifications
- Graduation from an accredited nursing school with a Registered Nurse license and five years of experience, or a Master's degree in Human Services with three years of relevant experience
- At least two years of applicable experience in case management or discharge planning in relevant settings
- Valid, active clinical license in North Carolina (LCSW, LMFT, LCAS, LCMHC, LPA, or RN)
- Preferred: Two years of supervisory experience and CM or CCM certification
- Knowledge of relevant laws, regulations, and best practices in clinical care and management
