Job Summary

A company is looking for a Supervisor II-Transition Coordinator.

Key Responsibilities
  • Supervise and develop staff to ensure compliance with policies and promote a positive work environment
  • Oversee unit operations, including the development of clinical protocols and departmental goals
  • Provide customer service by building strong relationships with providers and stakeholders
Required Qualifications
  • Graduation from an accredited nursing school with a Registered Nurse license and five years of experience, or a Master's degree in Human Services with three years of relevant experience
  • At least two years of applicable experience in case management or discharge planning in relevant settings
  • Valid, active clinical license in North Carolina (LCSW, LMFT, LCAS, LCMHC, LPA, or RN)
  • Preferred: Two years of supervisory experience and CM or CCM certification
  • Knowledge of relevant laws, regulations, and best practices in clinical care and management
