Key Responsibilities
- Collect and analyze healthcare data from telephonic contacts and EMR information
- Develop and implement care plans to promote patient health and safety
- Coordinate care delivery and evaluate progress towards patient outcomes
Required Qualifications, Training, and Education
- Graduate from an accredited School of Nursing; BSN preferred
- Current RN license or temporary license in the state of practice
- Basic Life Support for Healthcare Provider certification
- Additional education, training, or certifications may be required
- Physical ability to perform nursing tasks and respond to patient conditions
