Job Summary
A company is looking for a Triage Registered Nurse Coach.
Key Responsibilities
- Provide telephonic nurse triage to assess symptoms and determine appropriate care plans
- Act as after-hours clinical support for over 600 offices, responding to patient needs
- Collaborate with Care Connections Specialists and healthcare providers to coordinate patient care
Required Qualifications
- Bachelor's degree preferred
- Five years of clinical experience in a hospital, home health, ambulatory, or community setting
- Recent focus on acute and/or chronic-care management required
- Current unrestricted RN license for the states in the company's footprint required
