North Carolina Licensed Triage RN Coach

Job Summary

Providing essential after-hours clinical support, the full-time North Carolina Licensed Triage RN Coach will assess symptoms and guide patients through care decisions remotely, working three shifts a week from 7:00pm to 7:00am with occasional weekend shifts required.

Key responsibilities

• Provide telephonic nurse triage by assessing symptoms and determining the safest plan of care
• Act as the clinical resource for over 600 offices, responding to patient needs outside of primary clinic hours
• Collaborate with Care Connections Specialists and other healthcare providers to ensure continuity of care

Required qualifications

• Bachelor's degree preferred
• Five years of clinical experience in hospital, home health, ambulatory, or community settings with a focus on acute and/or chronic care management
• Current unrestricted RN license for the states in the Novant Health footprint
