Job Summary
A company is looking for a Post Transition Coordinator - QP to provide proactive intervention and coordination of services for individuals in the Transitions to Community Living program.
Key Responsibilities
- Support members in maintaining their homes and community inclusion through proactive planning and coordination
- Assist with rehousing efforts for members at risk of losing their housing or seeking new placements
- Document all interactions and interventions accurately in the Electronic Health Record and maintain compliance with relevant regulations
Required Qualifications, Training, and Education
- Bachelor's Degree in a Human Services field with two years of relevant experience, or a Bachelor's Degree in another field with four years of relevant experience
- Knowledge of MH/SU/IDD services and related regulations
- Proficiency in Microsoft Office products and relevant information systems
- Ability to work independently, manage multiple tasks, and adapt to changing priorities
- Residency in North Carolina or within 40 miles of the NC border is required
