North Carolina RN Care Manager

Job Summary

A company is looking for an RN Care Manager to develop and facilitate complex care management activities for members with physical needs.

Key Responsibilities

• Evaluate member needs and barriers to care, recommending plans for optimal outcomes
• Develop ongoing care plans and collaborate with providers to address unmet needs
• Coordinate care between members, families, and providers to ensure person-centered services

Required Qualifications

• Degree from an Accredited School of Nursing or Bachelor's degree in Nursing
• 2 - 4 years of related experience
• NICU experience required; maternity and care management experience preferred
• RN - Registered Nurse State Licensure and/or Compact State Licensure required
