Job Summary
A company is looking for a Telephonic Care Navigator.
Key Responsibilities:
- Conducts telephonic outreach to members and assists with scheduling provider appointments
- Educates members on care gaps and connects them with community resources
- Maintains adherence to metrics and ensures confidentiality of health information
Required Qualifications:
- High School Diploma / GED or equivalent work experience
- At least 1 year of call center or telephonic customer service experience
- 1 year of professional experience in an office setting using phone and computer
- 1 year of experience in healthcare, insurance, or community outreach
- Ability to work full-time and attend all training sessions without time off
