Job Summary

A company is looking for a Transition of Care (TOC) Coordinator.

Key Responsibilities
  • Manage the TOC Request to Move and Warm Hand Off processes across Trillium and other external agencies
  • Serve as the primary contact for providers, members, and internal departments regarding Benefit Plan Management and TOC inquiries
  • Oversee the timeliness of clinical reviews and track notifications for members or enrollees
Required Qualifications
  • High School Diploma/GED and six (6) years' experience in the BH/MH/SU/IDD field; OR
  • Associate's degree and four (4) years' experience in the BH/MH/SU/IDD field; OR
  • Bachelor's degree and two (2) years' experience in the BH/MH/SU/IDD field; OR
  • Equivalent combination of education/experience
  • Must have a valid driver's license and reside within North Carolina
