Job Summary
Providing proactive coordination of services for individuals transitioning from institutional settings, the full-time hourly North Carolina Transition Coordinator QP will manage caseloads, develop transition plans, and collaborate with community providers while working remotely and requiring travel within designated counties in North Carolina.
Key responsibilities
- Manage an active caseload and ensure successful transition planning for members/recipients
- Develop and implement comprehensive transition plans in collaboration with members/recipients and their support systems
- Document the transition process clearly and concisely to inform relevant stakeholders and comply with regulations
Required qualifications
- Bachelor's degree in a Human Services field or a non-human services field with four years of relevant experience
- Two years of post-bachelor's degree experience with the population served
- Knowledge of the MH/SU/IDD service array and Vaya Medicaid waivers
- Proficiency in Microsoft Office products and Vaya information systems
- Ability to maintain residency in North Carolina or within 40 miles of the NC border
