Job Summary

A company is looking for a Nurse Case Manager - RN (Remote U.S.).

Key Responsibilities:

• Contact and interview patients for health coaching programs
• Provide coaching to reduce high-risk behaviors and set achievable goals
• Conduct clinic visits, assess patient needs, and maintain medical records confidentiality

Required Qualifications:

• Active unrestricted RN Oregon State clinical license
• Graduation from an accredited Bachelor's Degree Nursing Program
• 1+ years of clinical and case management experience
• Medical record abstracting skills
• Proficiency in Microsoft Office and Internet/web navigation
