Job Summary

A company is looking for a Population Health Registered Nurse.

Key Responsibilities

• Conduct telephonic outreach for post-discharge assessments and educate patients on health screenings
• Perform chronic disease management and facilitate care coordination among healthcare services
• Collaborate with internal teams to improve population health outcomes and meet performance targets

Required Qualifications

• 2+ years of experience in case management or primary care with population health experience
• Engagement with Medicare and Medicaid populations with complex chronic conditions
• Nurse Licensure Compact (NLC) required; active RN license in specific states is a plus
• Ability to leverage motivational interviewing skills for patient behavioral changes
• Familiarity with Population Health and HEDIS quality gaps in care preferred
