Job Summary

Offering a flexible PRN opportunity, the Nurse Practitioner AZ License holder will conduct telehealth visits, perform clinical assessments, and coordinate care for patients, helping them access Community Health Worker services in a virtual environment.

Key responsibilities

• Conduct telehealth visits and perform comprehensive clinical assessments to identify health needs and social barriers to care
• Develop individualized care plans and provide medical clearance and referrals for Community Health Worker services
• Collaborate with Community Health Workers to support patient engagement and manage ongoing primary care needs

Required qualifications

• Active unrestricted Nurse Practitioner license with prescriptive authority
• Licensure in at least one of the following states: AZ, CA, HI, MI, AK, LA, NM, or NY; multistate compact licensure preferred
• Experience in primary care, urgent care, behavioral health, or chronic disease management; telehealth experience is a plus
• Strong understanding of social determinants of health and their impact on patient outcomes
• Fluency in Spanish or other languages is strongly preferred
