Nurse Practitioner Clinical Evaluator

Job Summary

A company is looking for a Nurse Practitioner- Clinical Evaluator & Chart Reviewer.

Key Responsibilities:

• Conduct in-depth evaluations via telehealth to assess weight management needs and overall clinical status
• Develop individualized care plans based on patient evaluations, ensuring they are evidence-based and goal-oriented
• Review patient charts and medical records to ensure accurate documentation of clinical findings and treatment plans

Qualifications:

• Graduate of an accredited Nurse Practitioner (NP) program
• Active, unrestricted Nurse Practitioner license (U.S.)
• Minimum of 2 years of clinical experience as a Nurse Practitioner, preferably in primary care or musculoskeletal settings
• Experience in chart review and developing comprehensive care plans
• Familiarity with regulatory requirements related to clinical documentation and care protocols
