Job Summary

A company is looking for a Remote Nurse Practitioner Contractor with experience in diabetes management.

Key Responsibilities:

• Provide video-based care to patients in a remote care program
• Evaluate patients' diagnosis, medications, and lab results to assess their eligibility for the program
• Support the clinical organization with clinical escalations as needed

Required Qualifications:

• Bachelor of Science degree in Nursing (BSN) and Master of Science in Nursing (MSN)
• Hold an unencumbered, active state license in at least 5 priority states
• Minimum 3+ years of experience managing patients with diabetes
• Spanish-speaking preferred, certification required
