Nurse Practitioner - NC and SC Licensed

Job Summary

A company is looking for a Nurse Practitioner for a National After-Hours Team.

Key Responsibilities

• Provide telephonic support and medical care to residents and facility staff after-hours
• Coordinate care using a telephonic platform between facilities, hospitals, and clinics
• Perform comprehensive assessments and document encounters in the EMR, ensuring compliance with CMS requirements

Required Qualifications

• Master's Degree as a Nurse Practitioner
• Current unrestricted NP license in NC and SC, with the ability to obtain in FL & GA within 60 days
• Nurse Practitioner national certification as ANP, FNP, or GNP
• 3+ years of experience as a Nurse Practitioner
• Background in acute and chronic disease management
